Future HSV-1 vaccine could avert over 40M infections, Qatar study finds

The findings come from a mathematical modeling study published in Science Advances, which offers the first population-level estimates of how an HSV-1 vaccine could reshape disease trends.

QATAR—Researchers at Weill Cornell Medicine-Qatar (WCM-Q) and Qatar University (QU) have found that a future vaccine against herpes simplex virus type 1 (HSV-1) could dramatically reduce infections and prevent tens of millions of cases over the coming decades.

The findings come from a mathematical modeling study published in Science Advances, which offers the first population-level estimates of how an HSV-1 vaccine could reshape disease trends.

HSV-1 remains one of the most widespread viral infections globally.

While it has long been linked to oral herpes, it is now increasingly responsible for genital herpes cases, particularly in high-income countries such as the United States.

The virus can also cause serious complications, including neonatal infections, encephalitis, corneal damage leading to blindness, and recurrent painful lesions affecting both oral and genital regions.

Shifting infection patterns and rising vulnerability

The researchers noted that reduced exposure during childhood has left more adolescents and young adults susceptible to infection later in life, especially through sexual transmission.

This shift has contributed to a growing burden of genital HSV-1, making prevention strategies more urgent.

To better understand the potential impact of vaccination, the team developed an age-structured mathematical model calibrated using four decades of data from the U.S. National Health and Nutrition Examination Surveys (NHANES).

They evaluated several vaccination strategies beginning in 2030, including infant immunization, adolescent catch-up campaigns, and booster programs.

Each scenario was tested under different assumptions about vaccine performance, reflecting the current direction of HSV vaccine research.

Modeling results show significant infection reduction

The projections showed that even a moderately effective vaccine could produce major public health gains.

Under a scenario with 70 percent efficacy and lifelong protection, infant vaccination alone reduced oral HSV-1 cases by nearly 50 percent and genital infections by about 60 percent by 2075.

This approach also prevented roughly 28.5 million infections over the study period.

When adolescent catch-up vaccination was added, the impact increased further, preventing an estimated 41.8 million infections.

The model also showed that the duration of protection played a critical role, as vaccines offering lifelong immunity performed about three times better than those lasting around 15 years.

Additionally, adolescent vaccination helped accelerate reductions in genital infections among young people.

Expert insights on vaccine development priorities

Dr. Bechir Naffeti of QU emphasized that vaccination could become a powerful tool for controlling both oral and genital HSV-1, especially if long-lasting immunity is achieved.

He noted that durable protection will be key to maximizing population-level benefits.

Dr. Houssein Ayoub added that strong quantitative modeling can guide vaccine development strategies by showing where interventions will have the greatest effect. He highlighted the importance of combining durable immunity with timely rollout and adolescent catch-up programs.

Dr. Laith Abu-Raddad of WCM-Q stressed that HSV infections remain a major global health challenge with limited prevention options, adding that advancing vaccine research is essential to reducing disease burden.

Research collaboration and ongoing scientific efforts

The study reflects a broader collaboration between WCM-Q and QU supported by the Qatar Research Development and Innovation Council.

While no new HSV-specific announcements have been reported in recent days from the institutions, both continue to advance work in infectious disease modeling and vaccine research through ongoing academic and regional health partnerships.
